Elevated gamma-glutamyltransferase (GGT~250) causes when all else normal?
For the last few blood tests, I have had elevated GGT enzymes of between 210 and 260. All other liver numbers and bloodwork are normal except total cholesterol is high, but only because HDL is quite high while LDL is quite low. Ratio is excellent. Blood sugar is all good. High HDL and elevated GGT are the only remarkable serological findings.
My doctor spoke to me about my drinking, assuming from the bloodwork that I was a drinker, and I agreed that I could cut back, but then we dug into my drinking and talked about what that actually means. He was kind of shocked to learn that I buy a six pack of beer for the weekend and it will usually last me two weeks, maybe a week if I have a guest over. I generally go for pints after work about every second Friday, but I rarely have more than two drinks. Maybe once a month or less I will indulge at a social event and have maybe 6 drinks.
My doctor said, “Looking at this GGT number, I thought you were going to tell me you were a two-or-three-bottles-of-wine-a-night guy.” I am not. I do tend to drink more wine when my wife is around, but we’ve been separated since mid July (and by “separated” I mean geographically, not romantically; thanks Covid!). Even when she was around, we’d open a bottle of wine with dinner and it would usually last two nights. Maybe on a Friday we’d indulge and kill the whole bottle. In short, in spite of the username (that was based on a one-time event), I am a somewhat regular weekend social drinker but generally low quantities.
I’m kind of at a loss to explain the high GGT count, but I don’t know if 250 is considered a little high or “better-get-a-biopsy” high. Certainly my doc thought it seemed high for the amount I drink. What other reasons would there be for high GGT when all else appears well within desirable ranges?
The only other relevant factor I can think of is that with each of the last three blood tests, I was actively recovering from athletic injuries and so was taking naproxen (220mg to 440mg 2 x daily) and etoricoxib (120mg daily, 3 x 7-day cycles, recently started on 90mg/day for a six-week initial period before review) to deal with significant inflammation. I am currently dealing with a full-thickness supraspinatus tendon tear and a broken toe, although the toe came after the last blood test.
I understand that some people may just have genetics that produce high GGT numbers. Any other ideas?
